It is one of the most rewarding parts of the job and also one of the most demanding. The difference between a hard shift and a good one often comes down to how you communicate and how well you understand what the person in front of you is experiencing. This dementia care guide for carers gives you practical tools you can use from your next shift.

What is dementia?

Dementia is not a single disease. It is an umbrella term for conditions that gradually affect memory, thinking, language, orientation and behaviour. It is not a normal part of ageing. The most common types are:

Alzheimer’s disease — the most common; usually starts with problems remembering recent events.

Vascular dementia — caused by reduced blood flow to the brain, for example after strokes.

Dementia with Lewy bodies — may bring visual hallucinations, fluctuating attention and movement problems.

Frontotemporal dementia — mainly affects personality, behaviour and language, sometimes at a younger age.

Signs you will notice in practice

Forgetting recent events while clearly remembering the distant past

Asking the same question several times

Not recognising the place, the time or sometimes familiar people

Struggling to find words or follow a conversation

Changes in mood: restlessness, anxiety, withdrawal or irritability

Difficulty with daily tasks such as dressing, eating or washing

Any sudden change — confusion that appears overnight, drowsiness or unusual agitation — should be reported straight away. It may be a sign of infection or another medical problem, not just the progression of dementia.

How to communicate with a person living with dementia

Approach from the front and say your name, even if you met this morning.

Use short sentences and ask one question at a time. Offer simple choices: “Would you like tea or juice?”

Give them time to answer. Silence does not mean they have not understood.

Do not argue or correct when the person is living in a different reality. Respond to the feeling: “You miss your mum. Tell me about her.”

Keep your tone calm and your body language open: eye contact, a smile, a gentle touch on the hand if they welcome it.

Use their life story: their job, family and favourite music open conversations and bring calm.

When distress or confusion appears

Behaviour is almost always a form of communication. Instead of asking “how do I stop this?”, ask “what are they trying to tell me?”. Check basic needs first: pain, hunger, thirst, the toilet, cold, noise or tiredness.

Many people become more restless in the late afternoon and evening, often called “sundowning”. Good lighting, a predictable routine, less noise and calm activities can help. If someone wants to “go home”, do not insist they are already home: walk together, talk about that home and gently redirect.

Food, hydration and safety

People with dementia often forget to drink: offer drinks frequently, in cups they recognise.

Small portions, finger foods and a plate in a contrasting colour make mealtimes easier.

A tidy, well-lit space without trip hazards reduces the risk of falls.

Record and report any change in appetite, sleep, behaviour or mobility.

Look after yourself too

Dementia care asks for a lot of patience and emotional energy. Talk to your team, use supervision, ask for support when you need it, and remember: you cannot control the illness, but you can make each moment gentler.

How Rafidin Education prepares you

Dementia awareness is part of Standard 9 of the Care Certificate, and employers value carers who arrive already prepared. Our CPD-accredited online courses help you understand dementia and communicate with confidence. You can also read our guide to person-centred care.

Dementia Awareness — types of dementia, communication and responding to distress.

Effective Communication in Care — practical techniques for difficult conversations.

Mental Health and Wellbeing Support — supporting the people you care for, and yourself.

End of Life Care — for the later stages, with dignity and compassion.

Study in English or Romanian, at your own pace, and receive your CPD certificate as soon as you finish.

Start today: see our Health and Social Care courses. Running a care home? We offer B2B training for your whole team — get in touch.

Frequently asked questions

Are dementia and Alzheimer’s the same thing?

No. Alzheimer’s is the most common type of dementia, but there are others, each with slightly different signs.

Should I correct a person with dementia when they get something wrong?

Usually not. Correcting can cause frustration and anxiety. It is more helpful to respond to the emotion behind the words and gently redirect the conversation.

Do I need a special course to work with people living with dementia?

Your employer will give you basic training, but a CPD course completed beforehand helps you feel ready from your first shift and looks great at interview.

What dementia is, how to communicate with the person you support and how to respond to distress.
